ENROLLMENT INTO THE EDCD WAIVER

2. Apply for Medicaid

  • In Person: Visit your local Department of Social Services (DSS) office.

3. Request a Screening

Once your Medicaid application is approved or if you already have Medicaid, you must request a screening for the EDCD Waiver program. This screening will determine whether you meet the criteria for needing long-term care services. The screening can be conducted by your local Department of Social Services or a hospital discharge planner if you are in a hospital.

4. Assessment and Approval
